(a NH consumer network) - Established in 2000, ServiceLink is a statewide network of locally administered community-based resources for seniors, adults with disabilities and their families. ServiceLink is a free information, referral, and assistance, with local offices in 13 communities and with many satellite offices throughout New Hampshire. ServiceLink answers questions and connects users to the appropriate services that support healthy and independent living.

 

 

 

 

 

 

Well Sense can help you get and stay healthy with complete medical, behavioral health, and prescription drug coverage. If you qualify for New Hampshire Medicaid, Well Sense Health Plan is here for you!

 

 

 

 

 

NH Healthy Families provides health care coverage for NH Medicaid. When you enroll with NH Healthy Families, they help you manage your health care needs in addition to paying providers for the health services they provide to you.

 

 

 

 

 

Provides families and health professionals with details of the care options available in the USA, as well as offering support along with your needs assessment.

 

 

 

 

 

New Hampshire's Electronic Application System (NH EASY) offers NH residents a fast and easy way to apply online for cash, medical, Child Care, Medicare Savings Program, and/or Food Stamp benefits since January of 2011.

 

 

 

 

 

The Caring for Your Parents Web site contains a wealth of online resources for people caring for aging parents, other relations, or friends. The site is also valuable for those who expect to become caregivers or are planning for their own future as they age.

 

 

 

 

 

Hospice Foundation of America provides leadership in the development and application of hospice and its philosophy of care with the goal of enhancing the U.S. health care system and the role of hospice within it.

Next Step in Care provides easy-to-use guides to help family caregivers and health care providers work closely together to plan and implement safe and smooth transitions for chronically or seriously ill patients.
